Our mission is simple: challenge archaic rules and implied medical conventions in healthcare to better serve patients and physicians.
Gradia is the backbone of the entire patient experience, helping patients get personalized, high-touch care while also making clinics more money. With our software patients can stay informed/educated about the status of their care, easily communicate with their providers, and get direct access to a specialized virtual care team.
Our software is currently at several clinics across the Southeastern US and helping thousands of patients get the care they deserve.
Founded by two Stanford CS students, Gradia is backed by YCombinator, Atlanta Ventures, Soma Capital, and incredible operators/founders of companies like Bolt, Ramp, Protocol Labs, and BillionToOne.
The Opportunity 📈
It’s an incredible time to join us: we’ve hit an infection point with fantastic patient engagement, tons of interest from physicians, and have recently unlocked massive revenue streams for our existing customers. However, we’re still early — there’s lots we want to change about the way healthcare currently works, so there are plenty of opportunities to contribute and make an outsized impact.
Your Role in the Company 💪
Gradia Health is looking to hire someone part-time (hourly pay & very flexible) who is a nurse practitioner (NP). This is a remote role and the typical hours per week will be 10-15 hours, starting out slowly and increasing over the first month.
What You'll Do
- Conduct virtual patient assessments, and screening for social determinants of health via telemedicine.
- Provide virtual medication management and reconciliation services to ensure medication adherence.
- Connect patients with community resources to address social determinants of health.
- Provide virtual patient education and counseling for chronic condition management.
- Collaborate with healthcare professionals for coordinated, patient-centered virtual care.
Requirements
- Active nurse practitioner licensure with at least 2 years of experience in direct patient care, preferably in remote/telemedicine settings.
- Experience in comprehensive patient assessments, individualized care plans, and medication management using telemedicine technologies for chronic conditions, especially digestive issues.
- Familiarity with electronic health record systems and compliance with healthcare documentation and privacy regulations in remote care settings.
- Proven ability to connect patients with community resources and provide virtual education and counseling to manage chronic conditions and address social determinants of health using secure telemedicine technologies.
