Our service is a virtual caregiver that monitors the instrumental activities of daily living (mobility, meals, groceries, medication delivery & home management) for tens of thousands of older adults around the United States. The company was started in 2016 and inspired by the cofounder’s grandmother, who still uses us to this day.

GoGoCare is a brand new line of service to provide GoGo clients with telephonic prescription management and care navigation support.

Job Summary:

As our first care team member for GoGoCare, you will play an important role in building out best program practices, executing deliverables, and assisting with the continued development of the program.

The Client Care Coordinator will be responsible for enrolling and onboarding new clients into the program and conducting prescription management and care navigation services over the phone. This role reports directly to the Healthcare Operations Manager and will work independently with limited supervision. 

The remote position, based in the United States only, is full-time (40 hours per week minimum) with hours between 8:00am to 5:00pm EST. This is a non-exempt position with an annual salary of $54,000-$58,000, depending on experience.

Responsibilities:

  • Serve as the primary point of contact for GoGoCare services.
  • Enroll new clients into GoGoCare.
  • Provide telephonic prescription management services to include
    • Work with clients’ providers and pharmacists to obtain timely medication refills.
    • Conduct medication assessments to make recommendations to providers. 
    • Educate clients on medications and medication side effects. 
    • Answer general medication questions.
  • Assess client needs and provide general care navigation and care coordination services.
  • Document client interactions and monitor client outcomes.
  • Assist in the development of program processes and best practices.
  • Collaborate with various team members including social workers, engineering, contact center representatives, etc.

Skills & Qualifications:

  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). License must be valid and in good standing. License in compact state or CA (preferred).
  • A minimum of 3-5 years of experience working with geriatric populations. Management experience (preferred).
  • Strong competence in medication management and care navigation.
  • Experience conducting clinical telephone assessments.
  • Strong customer service skills with experience conducting business, care coordination, and education, especially over the phone.
  • Excellent verbal and written communication skills.
  • Strong computer literacy, including experience with Electronic health records (EHR) and Google Workspace; familiarity with Slack (preferred).
  • Bonus: Spanish speaking a plus.
